How To Register Your Child For School


Parent or legal guardian MUST register the child.  Guardians and foster parents must provide proper court custody papers to register a child.  Many of the following items are required by law before your child is permitted to attend classes.  You can expedite your child's enrollment by having documentation and by planning ahead to create a smooth transition for your child.  No child may be dropped off at school without a parent/guardian completing the enrollment process.
  • Proof of Residency:  Purchase or lease agreement, rent or mortgage statement, utility bill or any bill with name and address on it, bank statement, auto insurance bill, or notarized Affidavit for proof of residency. (This form is provided by the school.)
  • Official birth certificate: A certified birth certificate is required (Code of Virginia 22.1-3.1). Hospital birth cards birth certificate cards, passports, etc. cannot be accepted. If not available, an Affidavit (form provided by the school) must be completed, signed and notarized. The Stafford Sheriff’s Office may be notified when a birth certificate is not provided within 90 days.
  • Social security number, if available
  • Proof of legal custody when applicable
  • A transfer slip or report card from a previous school
  • A copy of the current IEP and reports if the student has been receiving special education services 
  • A physical examination report completed within one year of school entry for grades K-5. Any students entering or returning to school from outside the United States must also furnish a physical examination done within one year. School Entrance Health Form MCH-213F
  •  School Entrance Health Information (form provided by the school)
  • Completed TB Risk Assessment Form (form provided by the school)

If your contact information has changed. . . . . . . . . .
 
It is imperative that the school have the most current contact information for your child.  If there is a change in your phone numbers, emergency contact persons, child custody issues, or address, please notify the school immediately.  In order for you to receive important information from the school (end of year report cards, summer school notification, newsletters, next school year class/team assignments, etc.), it is crucial that we have the most up-to-date address information.
 
If you are moving. . . . . . . . . .
 
If you will be moving at any time, please let the Counseling Office know as soon as possible so that we can update your child’s records.  If you will be moving within the Gayle Middle School attendance zone, you will need to provide proof of residence.  If you will be moving out of our school zone, please let the Counseling Office know so that we can begin the withdrawal process.  You will need to come to the school to fill out a withdrawal form and we will need to know where your child will be attending school.  The sooner that we are aware of the move, the sooner your child’s paperwork can be processed and mailed to the new school.  Request by parents to hand-carry records will only be honored for those students moving overseas.

Assistance for the Holiday Season and Winter Months

 

Dear Gayle Middle School Families,

 

We are fast approaching the holiday season and colder winter months.  In an effort to help ensure that all of our families have adequate food and warm clothing, we are sending home information concerning area resources that offer assistance. Working together, we can ease some of the challenges facing area families who may be struggling to make ends meet.

 

“Coats for Kids” is a program organized by a local church whose mission is to provide area students with warm coats.  Interested parents can call Mr. Deming at 540-373-0383 for more information. 

 

Shoe Fund” is a program through the Salvation Army that awards vouchers to purchase shoes for those students who need them. Please contact our school nurse, Mrs. Hamilton at 540-373-0383 for additional information.

 

Holiday Assistance

 

Gayle Middle School is joining with Stafford County Social Services and other local agencies to provide Holiday Assistance to members of the Gayle community. The Counseling Office will not be coordinating gift giving for Gayle families.

 

This year, all requests for Holiday Assistance

by Gayle families, or through Gayle Staff,

will be referred to The Holiday Assistance Team

of Stafford County Department of Social Services.

 

The DSS Team will coordinate with other local agencies

(e.g., SERVE, Salvation Army, Food Bank, individual churches, schools, etc.) to meet the needs of Stafford County residents as funds and in-kind gifts allow. They are cross-checking assistance requests in an effort to eliminate duplications.

 

If you are aware of a child in need of Holiday Assistance

please contact the Holiday Assistance Team at 658-8732.

 

If you wish to sponsor one or more children,

and you may specify the school and/or age of the children you want to sponsor,

please contact the Holiday Assistance Team at 658-8732.
