How To Register Your Child For School

Parent or legal guardian MUST register the child.  Guardians and foster parents must provide proper court custody papers to register a child.  Many of the following items are required by law before your child is permitted to attend classes.  You can expedite your child's enrollment by having documentation and by planning ahead to create a smooth transition for your child.  No child may be dropped off at school without a parent/guardian completing the enrollment process.
  • Proof of Residency:  Purchase or lease agreement, rent or mortgage statement, utility bill or any bill with name and address on it, bank statement, auto insurance bill, or notarized Affidavit for proof of residency. (This form is provided by the school.)
  • Official birth certificate: A certified birth certificate is required (Code of Virginia 22.1-3.1). Hospital birth cards birth certificate cards, passports, etc. cannot be accepted. If not available, an Affidavit (form provided by the school) must be completed, signed and notarized. The Stafford Sheriff’s Office may be notified when a birth certificate is not provided within 90 days.
  • Social security number, if available
  • Proof of legal custody when applicable
  • A transfer slip or report card from a previous school
  • A copy of the current IEP and reports if the student has been receiving special education services
  • A physical examination report completed within one year of school entry for grades K-5. Any students entering or returning to school from outside the United States must also furnish a physical examination done within one year. School Entrance Health Form MCH-213F
  •  School Entrance Health Information (form provided by the school)
  • Completed TB Risk Assessment Form (form provided by the school)
  • Evidence of the following immunizations (per Va. Immunization Code 32.1-46):  http://stafford.schoolfusion.us/modules/cms/pages.phtml?sessionid=e125ecb145466bd0487a5c092d6970b9&pageid=25796

If your contact information has changed. . . . . . . . . .

It is imperative that the school have the most current contact information for your child.  If there is a change in your phone numbers, emergency contact persons, child custody issues, or address, please notify the school immediately.  In order for you to receive important information from the school (end of year report cards, summer school notification, newsletters, next school year class/team assignments, etc.), it is crucial that we have the most up-to-date address information.

If you are moving. . . . . . . . . .

If you will be moving at any time, please let the Counseling Office know as soon as possible so that we can update your child’s records.  If you will be moving within the Gayle Middle School attendance zone, you will need to provide proof of residence.  If you will be moving out of our school zone, please let the Counseling Office know so that we can begin the withdrawal process.  You will need to come to the school to fill out a withdrawal form and we will need to know where your child will be attending school.  The sooner that we are aware of the move, the sooner your child’s paperwork can be processed and mailed to the new school.  Request by parents to hand-carry records will only be honored for those students moving overseas.

Counselor Visits to PE Classes
The counselors have completed visits to all the PE classes to review the dress code with students.  It has been an increasing concern this fall that some students are not adhering to the dress code as stipulated by SCPS and deliniated in the student agenda on page 12.  Of particular concern is the wearing of unauthorized head coverings, tank tops, sleaveless tops, sagging pants, short skirts, and pants with holes that reveal thigh skin or are a safety hazzard in some way.  Please review the SCPS Dress Code and be sure that students are dressed appropriately for school.

Stafford County Career Night
"Career Night" is for 7th, 8th, 9,th, and 10th Grade Students.  Please see the "Career Night Overview" link below for the details.

September Classroom Visits
The counselors have visited all classrooms/students in the 6th, 7th , and 8th grade. They introduced themselves to the students and talked with the students about, attendance, dress code, and cell phone policies.  The counselors and students discussed the importance of grades, why organization saves time, and why it is important to be prepared for class. The students had great responses and it was fun for the counselors to meet everyone. For the 7th and 8th grade students the counselors also talked about jobs/careers, how a career is related to their school work, and encouraged students to attend the county's “Career Exploration Night”.
Student Attendance Monitoring
The Counseling Department and the School Attendance Officer have worked together to notify parents of any unexcused absences their child has received to date.  Notes and attendance reports were sent home with students to parents asking parents to address these unexcused absences.  Parents are kindly reminded that notifying the school when their child is going to be absent and then following up with a written note upon their child's return to school is required.
Parent / Teacher Conference Night
Parents and guardians are encouraged to attend conferences on tuesday, October 13th from 4 - 7 PM.  While you are at Gayle, please stop by and see the counselors.  We will be in the area near the library.  We will be handing out information about tutoring, study skills, and lots more, so please stop by.
